FOR DENTAL PRACTICES

Answers for your dental team.

How do we help a patient get a home sleep test?

Start a referral through your practice account. The patient completes an online clinical questionnaire for review under Continuum’s medical protocols. Appropriate testing is physician-authorized before the patient proceeds.

Does our patient need a separate video appointment?

The asynchronous pathway uses an online clinical questionnaire, so a video appointment is not a routine step. Our medical team can request additional information when needed.

Can the results help with oral appliance treatment planning?

The physician-interpreted report provides findings and recommendations to help guide discussions about oral appliance therapy, PAP, or further evaluation. Your dental assessment remains part of determining suitability for an oral appliance.

Can our team track referrals and receive reports?

Yes. Your practice dashboard keeps patient progress and completed reports together so your team can coordinate follow-up.

Do we have to use Continuum Snapshot?

No. Snapshot is optional for practice partners. You can refer patients through the dental questionnaire pathway without a scan.

How do practice pricing and insurance work?

Contact Continuum for practice pricing and payment arrangements. We do not accept insurance at this time. Coverage for any subsequent dental treatment is separate from the sleep test.
