PRIVACY & SERVICE INFORMATION

Terms of Service

Website and clinical services

General website information and clinical services have different purposes. These terms work together with the applicable clinical consent. Reading educational content does not replace a personal clinical evaluation.

Eligibility and accounts

Direct-to-consumer testing begins with a brief eligibility screen, followed by an individual patient account for those who can continue. Completing the full clinical questionnaire does not guarantee authorization. Provide accurate information and protect your account credentials. Patients may access only their own records.

Orders and payment

Direct-to-consumer testing is self-pay; Continuum does not accept insurance at this time. The selected testing option and total price are displayed before checkout. Standard kit delivery is included. Core and Premier include up to three recording nights for the same package price, even if fewer nights are completed. Platinum includes one recording night of home polysomnography (PSG).

Testing and results

Follow device instructions and provide accurate information. Recording quality and clinical circumstances can affect whether a study is usable or further evaluation is needed. A sleep physician interprets the completed study and provides recommendations. Results and treatment outcomes vary.

Availability and support

The service is not for emergencies. Call 911 for a medical emergency. Service areas, availability, and support channels are described during checkout and in your patient account. Account changes do not prevent applicable access rights to clinical records.

Cancellation, returns, and additional charges

Cancellation, refunds, unopened and used kits, return shipping, lost or damaged devices, unsuccessful recordings, repeat testing, and any additional charges are confirmed at checkout and in the communications that accompany your order.
