HIPAA Notice of Privacy Practices

This notice

This notice describes how medical information about you may be used and disclosed, and how you can get access to this information. Please review it carefully. It applies to protected health information handled by the clinical provider in Continuum’s testing service. Website privacy practices are described in the Privacy Policy.

Uses and disclosures

Continuum and the clinical provider may use and disclose health information to evaluate eligibility, provide testing, interpret results, process orders, obtain payment, operate the service, and meet legal obligations. Health information shared with business associates is handled under appropriate arrangements.

Patient rights

You may have rights to access and request amendment of your health information, request confidential communications, request restrictions, and receive an accounting of certain disclosures. How to exercise those rights, and any limits, are available through the contacts below and in your patient account.

Complaints and contact

You may file a complaint with the clinical provider or with the U.S. Department of Health and Human Services if you believe your privacy rights have been violated. You will not be retaliated against for filing a complaint. Use the contacts in this notice or the Privacy Policy; do not include medical details in an unprotected public form.

Electronic delivery

This notice may be provided electronically as part of the clinical service. A website footer link is additional access, not a substitute for notice at the time of service. Accepting marketing terms is not acknowledgment of this notice or a blanket authorization to disclose health information.
